Admissions Checklist
For admission to the school, the following forms and documents must be submitted to the school office:

• application - Application Form (online)
• baptismal certificate, if Catholic
• birth certificate
• enrollment contract
• health/shot record
• most recent report card, if transfering from another school
• payment plan (if needed - bank draft form)
